Effective immediately, Job (Green) Cards will be issued only upon the submission, by fax (718-835-0896) or mail to Plumbers Local Union #1 (158-29 George Meany Boulevard, Howard Beach, New York 11414), of information contained in the attached form.

Job (Green) Card information will no longer be accepted over the telephone.
Additional copies of the Job (Green) Card Information Form may be obtained by visiting the Association’s web site (www.acpcny.org) under “Laborgram - Job (Green) Card.
